Supporting documents are optional for online submission. For an elected officer, the form asks for minutes documenting the vote; attach them if available. PDF, PNG, or JPEG; up to 3 files, 1 MB each, 2 MB total. Attachments are included in the email when you submit, not in your PDF download.
